As a patient, you have rights and responsibilities. Since we are a partner of the Smithville Hospital Authority, you may wish to read the Notice of Privacy Practices. Additionally, you will be provided with a current copy of our HIPAA Compliance Policies at your initial visit, as required by HIPAA and any other time, at your request.

You should also be aware of your responsibilities.*

1. Cancellations made less than 24 hours before your scheduled appointment may incur a charge.
2. Any outstanding balances must be paid at your appointment.
3. Any co-pays or amounts your insurance will not pay are your responsibility.
